Underwriting Requirements

Business Requirements

  • •DOT# and MC# (MC# required for intrastate risks only)
  • •3 years in business with continuous insurance
  • •U.S. garaging in a single state
  • •1–14 power units and up to 40 trailers
  • •For-hire trucking only
  • •No HOS or Unsafe Driver Alerts
  • •Less than 2 CAB alerts in other categories

Driver Requirements

  • •Valid U.S. driver's license
  • •Minimum age 23
  • •3 years verifiable driving history
  • •2 years CDL or similar vehicle experience
  • •Maximum of 3 minor violations OR 1 major + 2 minors (36 months)

  • •Hopper Bottom Trailers (Grain Only)

  • •Moving Floor / Conveyor Belt Trailers
  • •Livestock Trailers (Poultry/Small Animals Preferred)
  • •General Commodity Haulers
  • •Produce/Meat/Dairy Transport

Excluded Operations

The following risks are not eligible for coverage:

  • ✗Oilfield operations, loggers, hotshots
  • ✗Hazmat haulers requiring $5M limits
  • ✗Auto haulers, tow trucks, couriers
  • ✗Exotic animals, zoo, bees, or show animals
  • ✗Passenger transportation
  • ✗Household goods movers
  • ✗Mileage >125,000 miles per truck/year
  • ✗Vehicles older than 25 years (exceptions case-by-case)
  • ✗Drive-away operations
  • ✗Primary garaging outside the U.S.

Submission Requirements

  • 5–60 days before effective date
  • DOT and MC Numbers (MC for intrastate risks only)
  • Completed underwriting questions via FXM portal
  • Full vehicle schedule (Year, Make, Model, VIN, Stated Amount)
  • Driver schedule (Name, DL#, DOB, State Licensed, Hire Date)
  • Business owner's email address and date of birth
